The Utah Accident & Health Survey is a required
state specific annual filing. The purpose of this survey is to provide more
detailed information on Utah accident and health business than is currently
reported on the Utah State page of the NAIC Annual Statement.
Who is required to file?
Fraternal, Health, Life and Property & Casualty
insurers who report accident & health business in Utah on the Utah State
page of the NAIC Annual Statement are required to file this survey. All
other insurers are exempt. The completed survey is due March 1, 2008.
Failure to file by the deadline may subject your company to the enforcement
penalties under Utah Code Annotated § 31A-2-308.
Changes made in
the survey for 2007
Please note that survey has been revised for 2007.
These changes include new and updated sections for Medicare Supplement,
Medicare Advantage, and Medicare Part D Products and Long Term Care
business, as well as a new section on value-added benefits. The overall
format of the survey has been streamlined to make it easier to use.
Please review the changes and read this year’s instructions even if you
think you are familiar with the survey. This will save your time and
ours, and reduce the number of corrections after the survey is filed.
Also please remember each company is required to file a
complete survey. Answering with a “none” or “not applicable” when
appropriate is better than leaving a section blank. This includes providing
a contact person, with correct contact information, including a valid
telephone number and email address.
How to get the survey

using the links available below. The survey instructions are available in
Adobe PDF format. The survey form is available as a spreadsheet in Microsoft
Excel format.
Companies are expected to read the instructions before
