Instructions for CE Course Providers

Attached are:

• Copy of Rule R590-142 (all but Section 4);

• EXHIBIT A application for the approval of course  
                 credits (contact Michael Covington at
                 mcovington@utah.gov for the EX A and instructions to
                 become a CE provider); and

• EXHIBITS D, Course Roster.

• Continuing education fees:

• Effective November 1, 2003 Continuing Education total provider fees have now increased to $272.00 ($252.00 renewal + $20.00 E-Commerce fee)

• R590-102-11.  Continuing Education Fees.  

  • Continuing education provider approval fees:
      (a)  initial approval fee - due with application:  $252;
      (b)  renewal approval fee if renewed prior to renewal
            deadline - due with renewal application:  $252;
      (c)  renewal approval fee if renewed 1 through 60-days
            after renewal deadline and prior to approval lapse -
            due with renewal application:  $302; and
     (d)  Lapsed approval reinstatement fee if reinstated 61 days
            after renewal deadline - due with application for
            reinstatement:  $352.

  • R590-102-12.  E-commerce and internet technology services fee:
         Subsection (1)(c) states:
         "continuing education provider - due with the annual
          initial, annual renewal, or reinstatement application: $20."

  To become a continuing education provider, send an email to mcovington@utah.gov requesting the application and instructions.

  Checks should be made payable to the STATE OF UTAH INSURANCE DEPARTMENT.
