Health Discount Programs
Effective September 1, 2005, Health Discount Programs are required by Utah Law to be licensed and to renew on an annual basis. Below are the forms needed to do this. Questions should be directed to Brian.
These programs are not health insurance.
Purchasers of these programs are obligated to pay for all health care services. They will receive a discount for services rendered by those health care providers who have a contract with their Health Discount Program.
