If you would like to apply for a Certificate of Authority to do 
business in Utah please click on one of the following options 
to obtain the desired application (including instructions):

•   Utah Domestic Insurer Application (UCAA); or

 

•    A Uniform Foreign Insurer Application (UCAA).
